When applying for a certification transaction independent of a Georgia local unit of administration (LUA), the certification application must be completed through
your
MyPSC account. Additional certification forms are available at the links below.
All of these documents are in the Portable Document Format (.pdf). Forms must be printed, completed, and then scanned in order to submit an electronic copy.
